What's the skinny on the new Fitness Reward benefit?
We at Blue Cross Blue Shield of Massachusetts decided to scrap our previous fitness benefit and come up with a new and better way to reward you for all that huffing and puffing and crunching and climbing you do. We know getting and staying in shape is hard work, so we put the same energy into creating a plan that's no sweat to use. And for those of you who need a little extra push, here it is: The Fitness Reward. It's a $150 annual benefit that you can use toward membership or exercise classes at any health club you like. You can even use part or all of your $150 for your spouse or kids' health club fees. Okay, so we've given you motivation and a reward, but you'll still have to supply your own sneakers.
Who's eligible for the Fitness Reward?
Any subscriber to HMO Blue, Blue Choice, Blue Care Elect, or one of our employer-specific managed care plans can get the Fitness Reward benefit. Check with your human resources department to see if your company plan qualifies.
How do I get this fabulous benefit?
We couldn't have made it easier. (If you know a way, let us know!) You can get a claim form by clicking here or calling us at (800) 262-BLUE. Fill out the claim form, and mail it in with original receipts and a copy of your health club contract, and you're all set -- no sweat! (Until you get to your health club, that is.)
When should I send in my claim form?
Once you've been a member of your health club for at least four months in 2005, you can send in this year's Fitness Reward form once any time until March 31, 2006. When we get everything from you, we'll mail you a check in about two weeks.
What's considered a health club?
Grab your yellow pages and flip to Health Clubs. See the huge list there? That's what we cover, and more, including YMCAs and JCCs. It's that simple. (Your home gym doesn't count, though -- you can't use the benefit to buy workout equipment or clothing.)
Does it matter how I fork over the cash to my club?
No. You can pay it all up front or month to month, by cash, credit card, or electronic transfer, whatever works for you. Just keep a copy of your receipt and include the original with your claim form. We can't return any receipts or contracts, so please make sure you keep copies for yourself.
